COVID Vaccinations for ages 70-79 and the Clinically Extremely Vulnerable
In the coming week we will be inviting patients who fall into the above categories to attend for their COVID Vaccination via text, phone call or letter.
You will be offered an appointment at our Vaccination Site at Jubilee House in Leyland.
Please do not contact the surgery - you will receive your invitation in due course.

FLU CLINICS 2020
Our Flu Clinic dates are: Saturday 26th September, Saturday 3rd October, Saturday 10th October and Saturday 17th October
To be eligible to receive your flu vaccine at these clinics you must:
- Be aged 65+ or
- Be in the At Risk categories (including under 18s) or
- Be pregnant
Please note that patients aged 50-64 will NOT be vaccinated until
November/December pending further Government advic
You can book your appointment on the My GP App or by ringing the surgery
Monday to Friday between 2 and 5pm
Appropriate Social distancing measures have been put in place at these clinics

Coronavirus remains an ongoing risk despite the Government easing of restrictions and will continue to be a real risk to us all for the foreseeable future.
We need to continue to do everything we can to protect the vulnerable members of the community and protect your Surgery team so that we can continue to offer you an effective service.
Therefore for the foreseeable future the vast majority of appointments will be via telephone or video.
The ONLY exceptions are when an intervention is essential for your care. This would be for a physical intervention such as a blood test, injection or smear test or if a doctor or nurse has decided that a face-to-face assessment is essential in order to make a key decision on the next appropriate step.
Reducing such assessments is an important part of reducing the numbers attending the Surgery and thereby reducing the risk of virus transmission and keeping the Surgery as safe as possible for those who need to attend.
The safety of our patients remains a key priority, so we will continue to do what is necessary to achieve the right balance between keeping you away from the Surgery and asking you to attend.

GUIDANCE TO KEEP YOU SAFE WHEN ATTENDING YOUR VACCINATION APPOINTMENT
While coronavirus has put limitations on all our lives, it is important that you or your baby or child still have routine vaccinations. They protect against serious and potentially deadly illnesses and stop outbreaks in the community. We recommend that you [attend/book in for] your next scheduled appointment – see www.nhs.uk/vaccinations for details on when they are due.
Travelling to and from your appointment
When travelling to and from your appointment, please follow guidelines which include travelling by car, bike or on foot if possible, keeping a safe distance from others and washing your hands regularly. For more details, go to www.gov.uk and search ‘staying safe outside your home’.
When attending your appointment
When attending your appointment, we will be putting in place a range of measures to minimise any risk of COVID-19:
- social distancing measures will be observed;
- we may have asked you to attend your appointment at a clinic that is not at your usual venue;
- the appointment may take longer than usual.
Due to the ongoing response to COVID-19, our service may have a reduced number of appointment slots available. If you choose not to attend your appointment, please contact your GP practice. It is always helpful for us to know why so we can help you with any concerns you might have. If you still don’t wish to attend the appointment, it can be offered to someone else.
You must NOT attend an appointment if you or any member of your household are suffering from any of the symptoms associated with COVID-19 or are self-isolating. If this applies to you, please contact your GP practice so that you can reschedule your appointment for a different time.
If you are identified as clinically extremely vulnerable and have been shielding, please contact your GP practice for the latest advice on what to do.
Following the new NHS guidance, with effect from today, Monday 15 June, you are requested to wear a face covering when you attend the surgery.

We would like to reassure all patients that, whilst we are changing the way you access our services, we are not closed.
We would like to thank you for your understanding and patience. Although the phone lines and the surgery have been very busy today, you have been generally understanding of the new measures we have put in place.
We will keep you updated of any changes via the website and Facebook and would be grateful if you could keep your family and friends informed if they do not have online access.
We have made appointments for Vit B12 and Zoladex/Prostap available via telephone call to the surgery only.
You can help us to help you by following current Government health guidance for minor respiratory illness.
Access to the surgery is now via the side door. There is a bell to press and a member of staff will come and assist you.

Due to the current Coronavirus outbreak, as of Monday 16th March routine appointments with GPs, Nurse Practitioners, Practice Nurses and HealthCare Assistant are NOT being booked and the practice will run a telephone triage system only.
Our aim is to minimise exposure to Coronavirus and to ensure the safety of both our staff and patients. We are asking you NOT to come to the surgery unless asked to do so by a member of staff.
Any appointments that are currently booked will either be re-arranged or changed to a telephone appointment. YOU WILL BE CONTACTED BY THE PRACTICE IF YOUR APPOINTMENT IS SUBJECT TO CHANGE.

Information regarding Appointments
THE LEYLAND COLLABORATIVE
We have been working hard to increase the numbers of appointments available for you. This has involved developing a partnership with other local practices, which we are pleased to tell you has now resulted in the formulation of the group named above. The Leyland Collaborative consists of the following practices Worden Medical Centre, Central Park Surgery, Sandy Lane Surgery, and The Village Surgeries - Croston & Eccleston. The new service will provide access to GP and Nurse appointments at weekends and Bank Holidays ( except Christmas Day) from December 2017 and from January 2018, GP appointments weekday evenings. The service is available to all patients within this group on a proportionate basis however it is important to be aware that the appointment may not necessarily be at this practice. These are all additional appointments so it is not the case that patients of other practices will now utilise all our appointments.
Available appointments will be offered as normal through this surgery and you will be informed if the appointment is at another site.

Blood Test Results
If a clinician at the surgery has asked you to have blood tests, please remember it is the patient's responsibility to contact the surgery approximately 5 days after the bloods have been taken for the results. Reminder letters will no longer be sent out for repeat bloods due.
We have commenced using a text and email reminder service at the surgery. This will be used in many positive ways such as:
- Appointment confirmation and reminders. The patient can also cancel their appointment via text or email.
- Campaigns such as flu, shingles etc.
- Gathering data for eg: Friends and Family, smoking status.
- Sending out our Newsletter and patient questionnaires ( email )
For this service to be effective we need up to date mobile telephone numbers and email address. Please contact the surgery to update your contact details. You can use the Update Contact Form below.
